EMSI HIPAA compliant authorization forms for obtaining medical records are now available for download. These forms are not a substitute for facilities requiring special authorizations, however, they may facilitate EMSI's ability to obtain medical records in a more timely fashion. Please note: Facilities may not honor requests for medical records with authorizations that do not contain the eight core elements mandated under the HIPAA regulation guidelines effective April 14,2003.
